Kindly print out the relevant forms, complete the requested information and bring the completed forms with you to your scheduled appointment. Unanticipated circumstances present themselves from time to time, creating logistical challenges. As a courtesy to other patients, 24-hour advanced notice is required for all cancellations. In the event that a cancellation is made with less than 24-hour notice, or a patient does not show up to their scheduled appointment, the patient will be charged for the visit.
